Encompass Community Supports, formally known as Rappahannock Rapidan Community Services (RRCS) is hiring a Spanish speaking Support Coordinator II-Developmental Disability to work in our Culpeper office.

Our Support Coordinator II engages with individuals who have intellectual and other developmental disabilities to assess what supports would aid the person in living the life they desire. Support coordinators link the person to various services and resources, monitors the effectiveness of the services and coordinates among a support team. The supports take place in a variety of community settings work is performed under regular supervision.

At Encompass Community Supports, our full-time employees work 37.5 hours weekly.

SOME OF THE JOB RESPONSIBILITIES:

  • Facilitate, assess and evaluate client services and needs in the home, hospital or clinic
  • Facilitates, assesses, and evaluates client’s need for services at intake and / or for people receiving active case management, in a variety of community settings. Presents options and services based upon a needs assessment.
  • Develops an individual support plan appropriate to the evaluation with the client/family consensus. Reviews and updates support plans, when applicable.
  • Refers, links, and coordinates to multiple agencies to ensure appropriate supports, upon intake or for people receiving active support coordination.
  • Ensures programs operational compliance to all regulatory guidelines.
  • Prepares and maintains Electronic Health Records to document all activities performed with or on behalf of people receiving services.
  • Prepares applications, referrals for people to gain benefits or resources.
  • Advocates for clients’ human rightsand follows program guidelines to meet regulatory compliance.
  • Provides resources for crisis intervention and prevention services.
  • Attends meetings and conferences as they relate to client, staff, and program needs.

KNOWLEDGE, SKILLS AND ABILITIES:

  • Must be fluent in Spanish (ability to speak, read, and write in Spanish).
  • General knowledge of community and agency resources and programs of regional and state sources for the community service population.
  • General knowledge of interviewing and supportive counseling techniques.

REQUIRED QUALIFICATIONS:

  • Must be fluent in Spanish (ability to speak, read, and write in Spanish).
  • Bachelor’s Degree in Human Services.
  • Valid Virginia driver’s license
  • Employment with Encompass Community Supports is contingent upon the successful completion of our background check process (criminal, DMV, education, drug screening, reference check, DSS, OIG and SAM).

PREFERRED QUALIFICATIONS:

  • 1 year working with individuals with intellectual / developmental disabilities.
  • Case management experience
